Patients requesting a new appointment in the Division of Sleep Surgery should contact the appropriate new patient coordinator (listed below) for an appointment via telephone or e-mail (preferred). Any pertinent medical documents, including all previous sleep studies, must be faxed (415-353-2603) or mailed to the office for review. Once you are seen, we will promptly return a dictated letter and any other relevant information related to patient care to your referring providers.

Adult patients should bring the following with them to their appointment:

Please complete both forms in advance of your appointment, as they may require asking bed partners or family members about your sleep patterns.If possible, please complete these electronically and e-mail to Stephanie.Barrios@ucsfmedctr.org.
• Copies of full reports (not just 1-2 page summary, if possible) of all previous sleep studies
• Copies of operative reports from all previous surgeries related to the head and neck
