To refer patients for a consultation or second opinion in the Division of Sleep Surgery, providers should contact the appropriate new patient coordinator (listed below) for an appointment via telephone or e-mail (preferred). Any pertinent medical documents, including all previous sleep studies, may be faxed (415-353-2603) or mailed to the office for review. We will also ask patients to bring copies of all previous sleep studies and operative reports from all previous surgeries involving the head and neck. Once the patient is seen, we will promptly return a dictated letter and any other relevant information related to patient care.  Please contact us if you do not receive a dictated letter after a referred patient is seen.
